Memorial Plaque Order Form




PERMANENT MEMORIAL - OAK PARK TEMPLE

The Wall of Remembrance in the rotunda dates back to the Washington Boulevard Temple.  The plaques are in loving memory of those whom our congregants wish to memorialize.  On the Shabbat that the yahrzeit is observed, the names on these plaques are placed on the Book of Life wall near the sanctuary entrance.
